•Glucocorticoids are widely used in surgery to improve postoperative rehabilitation.•No data exist regarding effects of dexamethasone in head and neck cancer surgery.•More major complications occurred in patients receiving dexamethasone.•We do not support the use of dexamethasone in reconstructive head and neck surgery.
