Abstract
Given the large number of Canadians who die from cancer annually, it is important to consider palliative and end-of-life care as an integral part of a comprehensive cancer control strategy. The Canadian Partnership Against Cancer’s (CPAC) Person Centred Perspective Program identified Goals of Care with Advance Care Planning as a system gap that should be considered a key opportunity for improving quality and access to early and integrated palliative care. Based on this need, CPAC funded a multi-jurisdictional collaborative project to accelerate this work. The project is a partnership between CPAC, the Canadian Hospice Palliative Care Association and four provinces in Canada – Manitoba, Prince Edward Island, Newfoundland and Labrador and Nova Scotia. The overall goal is to create capacity within the four provincial cancer agencies/programs in launching goals of care and advance care planning policies and programs. By working together, the teams can optimize the development of cancer-specific tools and resources to be used with cancer agency partners. This interactive workshop will explore the work of this new project and the programs of each province. The workshop will share the experiences of the project partners and then encourage participants to discuss tools and resources, policies, processes, and evaluation mechanisms to implement advance care planning and goals of care programs in their own settings. The workshop is intended for anyone with knowledge of advance care planning or goals of care, cancer care, including health care professionals. Patients and/or family members are encouraged to attend to provide their unique perspective. The workshop will be facilitated by team members and will include short presentations from each province and a discussion with members of the audience.
