Klippel-Feil syndrome in association with Chiari zero malformation in the setting of acute traumatic spinal cord injury

•We report the first case of Klippel-Feil syndrome with a Chiari Zero malformation.•The patient presented with a traumatic spinal cord injury.•The patient was treated in a delayed fashion with posterior fossa decompression.•This is a reasonable treatment strategy in this setting.
