Abstract
Introduction: Overall, the incidence of colorectal cancer appears to be stable or diminishing. However, based on our regional cancer institute data, we observed the increasing incidence of colorectal cancer in patients under 40. Methods: The aim of this study was to verify the rising trend, clinicopathological feature and survival in colorectal cancer in patients under 40, in comparison to old age. Results: Young patients were 24% of total 1192 colorectal cancer (63% rectal & 37% colon). From 2004 to 2012 young population increasing. In young patients 59% were male and 41% were female. 9% young had family history & only 11% had history of alcohol intake. Onset of symptoms to reaching diagnosis period is longer in younger patients. (Range 3 months to 1 year in young, while 1 month to 6 months in older). Young adults were in a more advanced stage at presentation (70% in stage IV and 30% were in stage III, while in older patients 51%were in stage IV and 34% in stage III and remaining in stage I&II). There was a higher incidence of poorly differentiated tumor histology in the young patients (26% vs. 10% in the older patients) (p < 0.05). Eighty percent of all young cancer deaths were within 22 months (from 8months to 22 months). Conclusion: Above result show the different nature of carcinoma colorectal cancer and late diagnosis in young. These data highlight a need for thorough evaluation of young patients with colorectal symptoms, life style modification programme by govt. or non govt. organisation that include balanced diet of high fiber, low fat. Also early screening in high-risk individuals may improve young-onset CRC trends.
