Abstract
Introduction: The primary endpoint of this study was to know the incidence and treatment of gastric carcinoma in our area. Others secondary endpoints were the percentage of treated patients, the overall survival, the survival of the subgroups, and the more frequent treatments. Methods: Since 2010 to 2013 all the patients diagnosed with metastatic gastric adenocarcinoma and treated at the hospital Parc Taulí from Sabadell were registered. The clinical information was compiled and analyzed with chi2. The survivals were determined with Kaplan-Meier functions. Results: Out of a total of 168 patients, 79 patients (47%) metastatic at the diagnosis were studied, 44% women and 56% man, and they were 38-98 years of age. The localizations were 52% gastric body, 20% gastro-oesophageal junction, and 25% antrum. The OS was 5,05 months (95% CI, 2,99-7,10). 60% of the patients were treated with first line chemotherapy (CT). From them, 42% had a second line and 25% a third line. The DFS were respectively 6,62 months (4,06-9,17), 4,29 months (2,28-6,30), and 2,88 months (1,12-4,64) for every line of treatment. The OS of the patients that did receive chemotherapy was 9.7 months (6,40-13,00). Response (complete and partial) to the first line was achieved in 28% of patients; 29% had stable disease and 43% progression. The CT more used in first line were triplets of fluoropyrimidines, platinum and taxanes, a 45% (21 patients). Also just fluoropyrimidines and platinum without taxanes, a 38%. There is a difference in OS and PFS between first line CT with and without taxanes, favorable to include taxanes. As a second line the predominant chemotherapy is also platinum and fluoropirimidins (37%), and irinotecan (30%) In third line a 50% are combinations based in irinotecan. Conclusion: The OS of the patients who receive CT is significantly prolonged respect the ones who don't. Our results resemble the results as seen in the literature, even the more frequent chemotherapies.
