Quality of life and clinical effects among different urinary diversions following radical cystectomy

Objective:To compare quality of life among four types of urinary diversion(UD)after cystectomy in order to supply evidence of choosing suitable operation method.Method:We retrospectively reviewed our clinical experience of consecutive 101patients who underwent urinary diversions and bladder reconstructions after cystectomies from Jan.1995to Jun.2009.The data of 28patients with ileal neobladder(IN),22with sigmoid neobladder(SN),20with ureterocutaneostomy(CU)and 31with ileal conduit(IC)were evaluated by Functional Assessment of Cancer Therapy-Bladder(FACT-BL)and International Prostate Symptom Score(IPSS).Result: Seventy-eight patients were followed up successfully.There were significant differeces among different methods in operation time and postoperative hospital stay(P0.05).Differences of health-related quality of life(HRQOL) among four UD groups(IN,SN,CU,IC)were not significant(P0.05),but the socres of orthotopic neobladder(NB)in physical scale were higher.Conclusion:There were no significant differences in postoperative quality of life among four urinary diversions(IN,SN,CU,IC).Function recovery in body image of orthotopic NB is superior to incontinent UD(CU and IC).
