Simultaneous hepatectomy and splenectomy for primary hepatocellular carcinoma accompanied with liver cirrhosis and hypersplenism

Objective: To evaluate the clinical effect of simultaneous hepatectomy and splenectomy for primary hepatocellular carcinoma accompanied with liver cirrhosis and hypersplenism. Methods: Eighteen patients of simultaneous hepatectomy and splenectomy for primary hepatocellular carcinoma accompanied with liver cirrhosis and hypersplenism were analyzed retrospectively. Results: The overall complication and mortality were 72. 2%( 13 /18) and 11. 1%( 2 /18) respectively. Fourteen cases were followed up to the present time with a median follow-up of 23 months( ranges: 12 to 36 months). During the follow-up,five cases underwent tumor recurrence and two cases died. Conclusion: Combined liver and spleen resection,an operation with high risk,should be selectively applied in patients with primary hepatocellular carcinoma accompanied with liver cirrhosis and hypersplenism,for which combination consideration with preoperative examination and intraoperative exploration and individualized treatment should be taken.
