0238 : Echocardiographic analysis of myocardial deformation in hypertrophic myocardiopathy

Introduction Hypertrophic myocardiopathy is a genetic sarcomeric disease. Its severity is related to the risk of ventricular arrhythmias and sudden death. The application of a new echocardiographic tool, the 2D Strain, is being evaluated. The aims of our study were to investigate the global longitudinal Strain (GLS); Radial Strain (SR); Circumferential Strain (SC) of the left ventricle (LV) and the Longitudinal Strain (SL) of the right ventricle (RV) and to determine the value of these new parameters in the evaluation of the prognostic. Methods Itu0027s a prospective study of 105 patients with HCM. The conventional echocardiographic parameters, GLS, SR, SC and SL RV were studied. These data were compared to the measured parameters in 65 healthy subjects matched for age and sex. Results The mean age was 49,3±16,7 years with a male predominance (71,2%). The mean LV ejection fraction was 60,0±4.3%. The 2D Strain study revealed a significant decrease in all components of LV myocardial deformation in patients compared to controls. The GLS was –13,8±6,2% in CMH patients Vs –21,9±1,4% in controls (p –17,5% was correlated with the occurrence of syncope (OR=5,48; p=0,005). For an average follow-up of 32 months ± 6 months, the SL of the RV –17.5; Log rank p = 0,008). Conclusion Hypertrophic myocardiopathy is a disease of the whole myocardium with a biventricular alteration. The study by Strain 2D can detect subclinical impairment and has a contributive prognostic value for risk stratification of sudden death and guiding the therapeutic strategy .
