Abstract
Cornu cutaneum is an animal horn like, rare lesion, conical, protuberant, hyper- and parakeratotic disorder which occurs mostly on sun exposed skin surfaces. A 69 years old male patient is presented with a progressively growing keratotic lesion of the glans penis. Medical history contains circumcision, annuloplasty for mitral valve prolapse, spontaneous kidney stone passage several times. Hypertension, ischaemic cardiac disease and dyslipidaemia have been medically treated. Clinical examinations revealed a hyperkeratotic horn- like lesion of the glans penis. Surgical excision was performed with a rim of normal tissue; histology revealed invasive squamous cell carcinoma (pT2 Gr.I.) resected in toto. In the scar 2 recurrent lesions were observed 4 months after the excision of the primer tumour. After surgical resection no malignancy was found by pathological examination. Cutaneous horn is a keratin mass protuberance of the skin. These rare penile lesions are asymptomatic, hyperkeratotic and occur usually after 50 years of age. In approximately one- third of all cutaneous horns a premalignant or malignant lesion can be found on the base. Precise pathogenesis is unknown; diagnosis is made due to clinical aspect. Since there is a significant risk of malignancy a rapid surgical treatment is essential, additional therapy is defined by histopathology findings.
