count and gastric cancer recurrence after a R0 tumoral resection.PATIENTS AND METHOD: This retrospective study included 129 cases of gastric carcinomas, consecutively operated, with R0 resection.Exitus in the 30 first days and patients with liver cirrhosis, myeloproliferative disorders or acute inflammatory diseases were excluded.The relationship between preoperative thrombocytosis (Chi 2 test) (platelets > 400.000/µl) and the following variables was compared: gender, age >68, preoperative ASA, tumoral localization, size >5cms, total o partial gastrectomy, histological type, differentiation grade, vascular and perineural infiltration, serous infiltration (T4 vs T1-T3) and adenopathy invasion (N1-3 versus N0).Diseasefree survival curves (Kaplan-Meier-log rank) were determined by univariate analysis for each variable.Those with p<0.05 were included in a multivariate model [Cox model, stepwise, hazard ratio (HR) and 95% confidence interval (CI)].RESULTS: Preoperative thrombocytosis was registered in 14/129 cases (10.9%).Median postoperative follow-up was 63 months.Recurrence rate: 40/129 = 31%.: Serous invasion was the only variable associated to thrombocytosis: 71.4% versus 29.6 %; [OR = 5.96; CI 95% = (1.56-24.47);p = 0.002].Cases with thrombocytosis showed a higher disease-free survival curve: [HR = 3.03; CI 95% = (1.39-6.58);p < 0.001].Recurrence rate with normal platelet count: 1 year = 0.06; 2 years = 0.10; 3 years = 0.26; 4 years = 0.28.Recurrence rate with thrombocytosis: 1 year = 0.43; 2 years = 0.57, being all tumoral relapse diagnosed among the first 24 months.Time to recurrence: normal platelets: 21.8±13.3months; thrombocytosis = 10.2±6.8 (p = 0.024).Significance level of thrombocytosis was not maintained in the multivariate analysis [HR = 2.09; CI 95% = (0.94-4.66); p = 0.071].CONCLUSIONS: 1.-Preoperative elevation of platelets count is associated with a higher and earlier recurrence rate after a R0 gastric cancer resection.2.-Nevertheless, this predictive capability can not be considered as an independent prognostic value, may be due to its possible relation with a higher rate of serous invasion in cases with thrombocytosis.
