•Modest correlation exists between Doppler- and catheter-derived mLAP estimates in ACS patients.•Stronger agreement exists between the two techniques in the reduced EF sub-group.•LA size is an imperfect surrogate for mLAP in ACS patients.•In acute setting, echocardiographic evaluation of mLAP is reliable in ACS patients.
