Endometrioid Endometrial Cancer: Concordance Between Preoperative and Final Surgical Histopathology

Purpose To compare the diagnostic accuracy of histopathological reporting of endometrial sampling with the final hysterectomy specimen histopathology in clinical stage 1 FIGO grade 1/2 endometrioid-type endometrial cancer. Methods We retrospectively compared endometrial samples for preoperative histological type and grade with the final surgical pathology in 147 cases of FIGO grade 1 and 2 of endometrioid endometrial cancers, between April 2008 and March 2016. Results Out of total 73 grade 1 and 74 grade 2 cases, concordance was noted in 76.7 and 52.7% cases, respectively. 3.4% cases had a positive nodal status, and 18 cases were stratified to higher risk on the basis of other histopathological markers. Conclusion Discrepancies between preoperative and final pathology are known to exist, and preoperative endometrial sampling may be considered only a modest predictor of final histology and tumor biology in endometrial cancers, and hence, decisions regarding universal versus a selective surgical staging policy may vary between institutions and surgeons.
