•CYP3A5 polymorphisms did not impact i.v. tacrolimus exposure post-transplant.•CYP3A4 and ABCB1 polymorphisms impacted i.v. tacrolimus exposure post-transplant.•ABCB1 2677TT genotype was associated with increased risk of tacrolimus toxicity.•CYP3A4 and ABCB1 C2677T genotyping may help individualize i.v. tacrolimus dosing.•Pharmacogenomic guidelines should acknowledge differences in i.v. versus oral tacrolimus.
