•Patients with poorly differentiated tumors undergoing liver transplantation comprise a heterogeneous group of patients with different prognoses.•α-Fetoprotein concentration and tumor size can predict outcomes in patients with G3 tumors before liver transplantation.•A group of patients with acceptable survival can be selected from among patients with poorly differentiated hepatocellular carcinoma treated with liver transplantation.
