•Portal vein flow velocity and size are routinely determined before liver transplantation.•Clinically low portal vein velocity indicates severe liver cirrhosis, portal hypertension, and portal systemic collateral shunts.•Tolerability of inferior vena cava clamping depends on the presence of portal systemic shunts.•The correlation between preoperative portal vein flow velocity or size and changes in hemodynamics is weak. It cannot predict the tolerability of the inferior vena cava clamping in living donor liver transplantation.
