•We recommend a new pneumococcal vaccination scheme after allo-HSCT.•This scheme combines the benefits of early (≤6 months) and late (>1 year) vaccination.•Post-vaccination antibody measurements at regular time-intervals are important.•These can guide identification and possibly revaccination of non-responders.
