•The prevalence of perineural spread is a rare, but likely underappreciated, entity.•Abnormalities were found in the S1–S4 roots, lumbosacral plexus, and sciatic nerve.•Positive PET and MRI findings involving nerves suggest perineural spread.•Nerve biopsy should be considered in the presence of progressive neuropathy and positive imaging findings.
