•NLR it is useful to identify patients at highest risk for neck occult metastasis.•Significant relationship between NLR increments and occult metastatic node was found.•2.93 NLR value appears to be very close to the inflexion point of our curve.•2.93 NLR correspond to a 49.91% probability value of occult metastases in cN0 patients.
