•Diffusion tensor imaging (DTI) has good sensitivity to detect pathologic changes in cervical spondylotic myelopathy (CSM).•DTI metrics were significantly correlated with the neurologic severity in CSM patients.•These DTI cutoff values enable surgeons to predict surgical outcomes in CSM patients.
