•Iodine pseudoenhancement is inversely proportional to lesion size and enhancement.•Iodine pseudoenhancement is directly proportional to background attenuation.•Iodine pseudoenhancement values are similar in helical and axial scan modes.•Iodine pseudoenhancement can increase small lesion iodine levels to twice the true value.•Beam-hardening corrections in projection-based spectral CT can reduce iodine pseudoenhancement.
