•We measured decisional conflict in 174 patients with a lumbar herniated disc.•Nearly 50% of participants had clinically significant decisional conflict.•Decisional conflict was associated with lower patient satisfaction.•Discussing all available treatment options is likely to reduce decisional conflict.•Tools to reduce decisional conflict are needed for lumbar herniated disc.
