•PBC prevalence varies among studies and it is markedly increasing over time.•PBC is highly prevalent in Greece, with geographic clustering and peak during spring.•PBC in Greece is diagnosed at early stages, with males having a more advanced disease.•GLOBE score - a proposed mathematical model in PBC - has not been externally validated.•The GLOBE score has a high discriminative ability in Greek PBC patients.
