•Arterial wall elasticity (Eθ) reflects degrees of early-stage atherosclerosis.•We examined the association of cystatin C with Eθ in T2DM patients with IMT <1.1 mm.•Cystatin C was strongly and independently associated with arterial wall elasticity.•Serum cystatin C is a potentially useful marker of early-stage atherosclerosis.
