Abstract
•Refeeding syndrome (RFS) is a dangerous metabolic condition after nutritional replenishment if not recognized early and treated adequately.•RFS is most critical in the first 72-h period upon refeeding. Therefore, special attention must be given at the initiation of food replenishment.•According to the patient's individual risk for RFS, a careful start of nutritional therapy with a stepwise increase in energy and fluids goals and supplementation of electrolytes and vitamins is recommended.•We propose criteria for the diagnosis of imminent and manifest RFS.•An evidence-based algorithm to manage RFS in clinical practice is provided.
