•Extremely low level of serum PEDF was a specific, valuable, convenient and inexpensive biomarker for OI type Ⅵ patients.•Serum PEDF level did not provide information regarding the clinical severity of OI and the efficacy of bisphosphonates.•Weight was an independent predictor of serum PEDF levels in OI patients.
