Since its first description as "carcinoid" more than100 years ago, neuroendocrine neoplasms (NEN) have had several name changes and it took a long time until an officially accepted grading system as well as a TNM classification were introduced in 2010. Although these have been well accepted and proved to work in practice, some changes were necessary in the new 4th WHO edition on endocrine tumours as well as in the 8th TNM classification of tumours. In this review the most important changes are summarized. They include: 1. Introduction of a new category "NET G3" for highly proliferative NEN with well differentiated morphology, 2. Replacement of the term MANEC with MiNEN for mixed tumours, and 3. Unification of the ENETS-, UICC- and AJCC-TNM classifications. Furthermore, it is referred to the periodically updated diagnostic and therapeutic recommendations of the European Neuroendocrine Tumor Society (ENETS) and a paragraph is addressing the importance of reporting NEN patients to the national registry SwissNET.
