•Pneumorrachis is characterized by the presence of air within the spinal canal.•The causes of pneumorrachis can be divided as iatrogenic, non-traumatic and traumatic.•Only eleven previous cases have been reported and we describe two additional.•It is a marker of severe injury and the majority of such patients have a poor outcome.•Although pneumorrachis is usually asymptomatic, prompt recognition of the underlying cause is essential.
