摘要
To describe the effect of ulipristal acetate (UPA) on fibroids characteristics assessed with 3D ultrasound. A prospective observational study was designed in a university hospital, including premenopausal women with symptomatic uterine fibroids who received 12 weeks of 5 mg/24 h UPA treatment. A 3D ultrasound was performed pre and post UPA treatment by a single gynecologist (Voluson V750 Expert, GE). Variables considered were fibroids number, volume (using VOCAL™ software) and vascularisation of fibroids (subjective score 1–4), uterine volume, antral follicle count (AFC), endometrial type and endometrial thickness. 19 women were included in the study. Median age was 42.76 years (IQR: 39.1–47.5) and median number of fibroids was 2 (IQR:1–8). All patients had abnormal uterine bleeding and 5 out of 19 (26.3%) had symptoms of compression. No differences were found on fibroids volume or vascularisation, on uterine volume, AFC neither on endometrial thickness (table 1). Thirteen out of 19 patients (68.42%) reported amenorrhea (5 patients) or decreased bleeding (8 patients), while 5 women reported no changes in menstruation and only one expressed an increase in bleeding. In this preliminary study, after 12 weeks of treatment with UPA no differences were found in ultrasound characteristics of fibroids, uterus volumen, endometrium and ovaries but most of the patients reported an improvement of their bleeding.
