OP09.11: Third trimester abnormal uterine artery Doppler is associated with adverse pregnancy outcome

To evaluate the significance of abnormal uterine artery (UtA) Doppler in third trimester among patients with and without intra uterine growth restriction (IUGR). A prospective study was designed. 198 consecutive singleton pregnancies between 28–40 weeks of gestation were included. 80 women had IUGR and preeclampsia. Of those 9 had abnormal UtA Doppler. 118 women were control group. Of those, 8 women had abnormal UtA Doppler. UtA Doppler was considered abnormal when bilateral notch or pulsatility index above the 90th% were noticed. Gestational age at delivery was significantly lower among patients with abnormal UtA Doppler in comparison to normal UtA Doppler, in either group: patients without pre-eclampsia or with pre-eclampsia (35.4 vs. 37.6 weeks, P > 0.04; 34.6 vs. 37.6 weeks, P < 0.01, respectively). Also, neonates with abnormal UtA Doppler had significantly: 1) Higher rates of Apgar scores < 7 at 5 minutes in both groups (9.8% vs. 2.3%, P − 0.02; 8.5% vs. 4.3%, P < 0.04) 2). Significant lower pH at birth (7.21 vs. 7.33, P − 0.02; 7.20 vs. 7.31, P − 0.02). Birth weight and rate of Cesarean section were not significantly different between patients with and without abnormal UtA velocimetry. Third trimester abnormal UtA velocimetry is associated with higher perinatal morbidity, both among patients with and without pre-eclampsia.
