Objective: proof-of-concept study to define immunoscore in bladder cancer studying Th1immuno-reaction. METHODS From 10/2005 to 04/2010 252 patients underwent primary TURB-T at our Institution. Retrospective review of a 30 patients selected cohort with pT1 and Muscle Invasive Bladder Cancer (MIBC) lesions was performed. Clinico-pathological features and follow-up were collected. Pathology blocks were marked with CD3 (2GV6, Ventana) and CD8 (C8/144, Dako). Immune cells density in stroma reaction (SR) was measured on 5 high power field (HPF) by a dedicated uro-pathologist blinded for patient's evolution. Statistics: Pearson coefficient to test correlation, Kolmogorov-Smirnov and Shapiro-Wilk tests for normal repartition of data. Student test or non-parametric Wilcoxon test as appropriate to compare mean between 2 groups. Log-rank test to compare survival probability. Univariate Cox model to assess survival's predictors. RESULTS Clinico-pathological characteristics are presented in Table 1. 10 pT1 cases and 20 MIBC cases were analyzed. Immunohistological analysis for pT1 lesions featured limited SR. For MIBC, mean density of lymphocytes in the SR was of 105/HPF (CD3) and 86/HPF (CD8). Lymphocytes T3 and T8's mean densities were strongly correlated to each other (p=0.0001). Survivors harbored higher lymphocytes densities vs non survivors (T3: p=0.0319; T8: p=0.0279). Patients harboring low T3 (p=0.0294) and T8 (p=0.0294) tumor's densities had significant higher mortality on survival analysis. T3 (p=0.026) and T8 (p=0.020) lymphocytes densities were independently associated with cancer-specific survival. CONCLUSIONS High T3 and T8 lymphocytes stroma reaction densities are associated with better cancer specific survival for =>pT2 lesions. Th1 immune reaction to the tumor seems to be protective for bladder cancer. Further evaluation is warranted. Fig 1. Survival curve high (blue) vs low (red) LT8 densities.
