1477 FIVE YEAR OUTCOMES OF THE CRYO ONLINE DATA REGISTRY: PRIMARY WHOLE GLAND POPULATION

INTRODUCTION AND OBJECTIVES: The COLD Registry is the largest existing data base of cryosurgical outcomes. Herein we report a comprehensive update of the outcomes for patients with minimum 5-year recorded data. METHODS: A risk stratified cohort of 1,280 treatment naive patients treated with primary whole gland cryoablation was identified. Nadir and follow up PSA levels were studied to assess biochemical progression free survival (bPFS) based upon Phoenix criteria. Specific PSA range ( 0.1ng/ml, 0.1 ng/ml-0.5 ng/ml, 0.6 ng/ml-1.0 ng/ml, 1.0 ng/ml-2.5 ng/ml) Kaplan Meier curves were constructed and populated with 5 year patient data. Procedure related complications are reported. RESULTS: Mean age of the cohort was 70 7.05 years. PSA at diagnosis: 4.0 ng/ml (n 204), 4-10 ng/ml (n 840), 10 20 ng/ml (n 167), 20 ng/ml (n 46). Gleason distribution: 6 (n 816), 7 (n 322), 8 (n 440). Risk stratified biochemical progression free survival for specific PSA ranges is depicted in Table 1. 49 patients had nadir PSA 2.5 ng/ml (3.8%), 34 (69%) of whom demonstrated biochemical progression within 18 months. Continence was preserved in 96.6% of patients, while 1.5% of patients had retention at 12 months. 33% of patients were potent at 12 months. CONCLUSIONS: 73% of patients achieved a nadir PSA 0.6 ng/ml which correlated with 83.3% 5 year bPFS. Failure to achieve a nadir PSA 0.6 ng/ml predicts a high likelihood of eventual clinical failure and suggests the need to consider further intervention.
