摘要
s26 others (mean age 26; 50% male; 73% white; baseline 92 eos/hpf; post-steroid 85 eos/ hpf).A total of 13 (50%) responded to a second-line treatment: fluticasone was changed to budesonide in 6 patients and 2 responded (33%); compounded budesonide was used in 2 and 1 responded (50%); the dose of topical steroid was increased in 14 and 2 responded (14%); ciclesonide was used in 3 and none responded; prednisone was used in 5 and 1 responded (20%); montelukast was used in 7 and 1 responded (14%); 6MP was used in one without response; and 16 had dietary therapy and 6 responded (38%).Concomitant esophageal dilation was performed in 13 of 26 patients (50%), and was associated with symptomatic, but not histologic, response.Overall, the mean number of secondary treatments prescribed was 2.1 ± 1.5.Conclusions: No second-line medical therapy for refractory EoE achieved a >50% response rate. While half of EoE patients refractory to topical steroid therapy ultimately achieved histologic remission, multiple trials of second-line treatments were necessary. The choice of second-line treatment should consider individual patient factors and potential adverse effects.
