I appreciate Dr. Cohen's comments on our article. A comprehensive description of the cutaneous paraneoplastic syndromes associated with lung cancer is useful, although beyond the scope of the article. The list that Dr. Cohen has provided is helpful for reminding physicians to maintain a high index of suspicion for the protean manifestations of lung cancer at initial diagnosis or as an indication of metastatic growth or recurrence of disease. Although the pathogenesis of many of these dermatoses remains undetermined, a postulated mechanism is that they are caused directly by a cytokine secreted by the tumor; alternatively, the neoplasm may participate indirectly by inducing accessory cells to secrete the causative factor.
