•We model the determinants of ambulance running costs in Norway.•We find that demographic factors are importing drivers of demand.•Accessibility patterns are the key driver of running costs.•Population ageing will act to increase demand and costs.•Continuing urbanisation will act to reduce costs of running ambulance services.
