Dos años de seguimiento en la revascularización de extremidades inferiores basada en ultrasonografía

Introduction: To compare the outcome of lower extremity revascularization with great saphenous vein (GSV) based exclusively on arterial Doppler mapping versus other diagnostic methods. Material and methods: A total of 91 patients were selected and followed-up for 2 years from the period November 2009 until December 2011, with the inclusion criteria; GSV bypass and no previous revascularization. Two groups were established; group A (n = 32) using only ultrasound Doppler as pre-surgical planning test and group B (n = 59) using other diagnostic methods. A historical cohort study was conducted, in which the results were compared (primary patency, secondary patency, major amputation-free rate and survival rate) in both groups. Statistical analyses were performed using survival curves of Kaplan-Meier and log-rank statistic. Results: The results in group A and B, respectively, in terms of: primary patency was 84% (95% CI: 72 to 96) vs. 76% (95% CI: 65 to 88); secondary patency was 93% (95% Cl: 87 to 99) vs. 85% (95% CI: 76 to 95); the free of major amputation rate was 93% (95% CI: 87 to 99) vs. 92% (95% CI: 86 to 99), and the survival rate was 96% (95% CI: 90 to 100) vs. 85% (95% Cl: 76 to 95). The results were also compared according to the receiver vessel (popliteal or distal) and as a function of the risk factors, with no significant differences. Conclusion: The study shows that long term revascularization, based on a high reliability ultrasound test, has the same permeability as other imaging studies. (C) 2014 SEACV.
