•Healthcare technology adoption can be explained by ‘technology identities’.•Identities are attributes with which a technology becomes imbued by stakeholders.•CRP POCT non-adoption is due to weak utility, financial risk and pragmatic issues.•NPT INR was adopted for utility (service redesign) and effectiveness over cost.•INR self-testing had not been promoted due to perceived high clinical risk.
