A Case of Communicating Scrotal Edema Diagnosed by 3 Dimensional-Computed Tomographic Peritoneography in a Patient on Continuous Ambulatory Peritoneal Dialysis and Improved by Conservative Treatment

Abstract
症例は糖尿病性腎症を原疾患とする慢性腎不全末期の56歳,男性である.2004年3月22日に持続携行式腹膜透析(continuous ambulatory peritoneal dialysis;CAPD)を導入し合併症なく退院したが,5月2日より陰嚢腫大が出現し,同時に除水不良が顕著となり約8kgの体重増加を認めた.Computed tomographic peritoneography(CT peritoneography)を施行し再構成した3次元画像を元に腹膜鞘状突起の開存による交通性陰嚢水腫と診断した.陰嚢水腫と除水不良は1.5%ブドウ糖透析液の注液量を2.0Lから1.5Lへの減量と安静のみで改善した.患者の強い希望によりその後も手術を行わず経過観察したが2005年11月8日に永眠されるまでの19か月の間に交通性陰嚢水腫の再発は認められなかった.CAPD患者の交通性陰嚢水腫の診断においてCT peritoneographyの有用性を報告している文献は散見されるが,3 dimensional-CT peritoneography(3D-CT peritoneography)は解剖学的関係をより明確に示すことができる信頼性の高い検査であることが示された.また,CAPD患者の交通性陰嚢水腫を保存的に治療した症例は稀である.
