•In 32.4% the GP underestimated a patients’ CHD risk, compared with the UKPDS score.•Male gender, smoking and cholesterol level were associated with underestimation.•Getting aware of their underestimation may enhance GPs to intensify treatment.•After 1 year, CHD risk relatively improved in patients with an underestimated risk.
