•We systematically analyzed data on the effects of RAPA on Treg cells.•RAPA appeared to maintain Treg cells better than CNI in transplant recipients.•The effect of RAPA on Treg cells might be time-dependent.•Treg cells in different proliferative stages have varying sensitivities to RAPA.•The possible mechanism was also discussed in our review.
