Posterior SMA Syndrome following subcortical stroke: contralateral akinesia reversed by visual feedback.

•Stroke-induced SMA-basal ganglia disconnection causes contralateral premotor deficit.•Decrease of motor speed and initiation, termed akinesia, are part of SMA syndrome.•Akinesa was partially corrected under external cue by visual control.•This case shows the role of SMA in internally cued motor initiation and maintenance.
