Dear Editor, Kidney transplantation is the treatment of choice for end-stage renal failure (ESRF). However, there is a universal shortage of donor organs and increasing waiting lists. Accepting donors from extremes of age is one way of addressing this disparity. The transplantation of pediatric kidneys into adult recipients has become an acceptable choice as an expanded criterion transplant (1). Pediatric en bloc kidney transplantation (EBK) was developed to address the concerns regarding suboptimal nephron mass and increased incidence of vascular thromboses in pediatric single-kidney transplants into adult recipients (1). We report a case of pediatric EBK with only one HLA match and review the literature with regard to the issues involved.
