Abstract
Application of adjuvant isolated limb perfusion to excision of primary melanoma cannot be recommended, although it may provide valuable locoregional disease control in patients who have frequently recurrent resectable locoregional melanoma. Unresectable limb melanoma is the primary indication for perfusion, with better response rates when tumor necrosis factor alpha is used in patients who have a high tumor load. Repeat perfusion can be performed. The long-term health-related quality of life of survivors of melanoma who underwent treatment with perfusion is comparable to that of their healthy peers.
