患者男,45岁.主诉为发作性多次短暂意识丧失1年.神经系统检查正常,头部CT和MRI检查正常.入院后行弓上DSA检查,显示双侧椎动脉起始部缺如.左颈总动脉造影证实吻合血管起源于左颈内动脉第三椎体水平.左椎动脉水平部由一个起源于左颈内动脉的血管组成,此血管上行至寰枕连接处后背曲经枕大孔入颅(图1).此血管供应基底动脉和大脑后动脉.
