Avaliação da Função Diastólica após o Transplante Autólogo, Intracoronário, de Células Mononucleares da Medula Óssea em Pacientes Submetidos à Angioplastia Primária Left Ventricle Diastolic Function after Intracoronary Autologous Bone Marrow Mononuclear Cells Transplantation in Patients Treated for Primary Angioplasty

Objective: Evaluate the effects of intracoronary autologous bone marrow mononuclear cells transplantation (ABMMCT) over left ventricle diastolic function (LVDF) by using Tissue Doppler imaging (TD) Echocardiography . Methods: Open label concealed randomized clinical trial. Nine pts (7 male), were submitted to successful percutaneous coronary intervention (PCI) for ST elevation acute myocardial infarction from the outlet of this study. Patients were enrolled in the protocol if they had wall motion abnormalities on echocardiography and a fixed perfusion defect on the single- photon emission computed tomography (SPECT) in the related coronary artery. Patients were followed for 3 months and randomly assigned in 2 groups: treated with ABMMCT (TG, n=6) and a control group (CG, n=3). Echocardiogram were digitally stored on Vivid 7 and evaluated in a blinded way. The following parameters were important: pattern of diastolic mitral inflow (DMI), E peak, A peak, and E/A relation. At DT were examined at mitral annulus E' peak and A' peak on lateral (E' lat and A'lat) and septal (E's and A's) walls, E'lat/ A'lat relation, E's/A's relation and E/E'lat (E/E'). Results: At TG, DMI turned into normal in 2 pts. At CG had worsened of DMI in 1 pt and lack of improvement in the other two. At TDI, lateral mitral annulus LVDF normalized in 3 pts at TG, and had no improvement at CG. At TDI, septal mitral annulus LVDF normalized in 2 pts at TG, and had no improvement at CG. E'lat shortened at CG and increased at TG. E'lat/A'lat relation became unaltered at CG, and increased at TG. E/E' relation increased in CG and was reduced at TG. Conclusions: In this group of pts, LVDF, analyzed by DMI and TDI, showed improvement in the TG in comparison with CG.
