Abstract No. 246: Pregnancy and vascular anomalies: What are the risks and outcome?

To determine maternal and perinatal morbidity in women having vascular anomalies. All pregnant women in our Multidisciplinary Clinic of Vascular Anomalies were included in this study. We reviewed the maternal demographic data: age, type of vascular anomalies, number of pregnancies, coagulogram, medical history. The impact of the pregnancy on the evolution of vascular anomalies was assessed using the following scale: no effect, volume increase, pain increase, bleeding. Prenatal morbidity was analysed using the following parameters: biometry, intrauterine growth restriction, child outcome. Our series included 11 mothers: Klippel-Trenaunay (KT) with pelvic involvement (n=5) (# of pregnancies: 6), venous malformation of the pelvis (n=1) (# of pregnancies: 1), head and neck lymphatic malformations (n=2) (# of pregnancies: 5), head and neck venous malformation (n=1) (# of pregnancies: 1), arteriovenous malformations (n=2) (face n=1, hand n=1) (# of pregnancies: 3). Except for the 2 patients with lymphatic malformations, elevated fibrinogen and D-Dimer levels were observed in all mothers. On the 16 pregnancies conducted at term, no effect on vascular anomalies was observed in 3 pregnancies (1 KT and 2 lymphatic malformations), a moderate volume increase was noticed in 2 (1 KT and 1 lymphatic malformation), whereas important volume increase was seen in 10 patients: arteriovenous malformations (n=2) (# of pregnancies: 3), KT (n=4), lymphatic malformations (n=3). Only one patient with a KT had a severe complication (paresia of right leg secondary to spine involvement). No intrauterine growth restriction was observed during the prenatal period. All but one baby were delivered vaginally (1 c-section). The Apgar score was normal in all babies. All vascular anomalies decreased to their baseline volume 3 to 6 months postdelivery. Pregnancy has an impact on the evolution of vascular anomalies. Nevertheless, the effects are unpredictable. However, a return to baseline condition can be expected after delivery. No impact on fetal growth was observed in this series.
