~(18)F-FDG PET/CT显像在非小细胞肺癌淋巴结转移及分期中的应用

Abstract
目的对比~(18)F-FDG PET/CT 和 CT 评价非小细胞肺癌(NSCLC)患者淋巴结转移及分期(N 分期)的价值。方法回顾性分析68例行根治性手术治疗的 NSCLC 患者资料(术前同期行增强 CT 和~(18)F-FDG PET/CT 检查,术中清扫各站淋巴结),根据其术后病理检查结果,结合影像学诊断,比较 CT 和 PET/CT 对肺癌淋巴结转移的诊断价值及确定 N 分期的准确性。结果 CT 和 PET/CT 诊断纵隔淋巴结转移的灵敏度、特异性、准确性、阳性预测值(PPV)和阴性预测值(NPV)分别为63.6%,64.6%,64.3%,47.7%,77.8%与87.9%,87.7%,87.8%,78.4%,93.4%,差异均有统计学意义(P<0.05);CT 与 PET/CT 诊断 N 分期的准确性分别为54.4%和76.5%,两者差异有统计学意义(P<0.05)。结论 ~(18)F-FDG PET/CT 在诊断 NSCLC 纵隔淋巴结转移方面的临床价值高于 CT,能够更准确地确定 N 分期,指导治疗方案的选择。
