P696* Aims: We investigated the prognosis of 11 children underwent ABO-incompatible renal transplantation in our institution. Methods: 11 children (7 girls and 4 boys) under 18 years old were performed ABO-incompatible living related renal transplantation from Aug.1995 to Nov. 2002 in our institution. After plasma exchanges to eliminate anti A and B antibody before operation, splenectomy was done fore all patients during transplant surgery. We studied the incidence of acute rejection within 3 months, graft survival, patient survival of these 11 recipients retrospectively. Results: Mean age and body weight of recipients was 13.18±4.08 years old (range: 6.31-18.89), and 29.28±13.72kg (range:14.00-55.00) respectively. Causes of renal failure were FSGS (n=4), hypo/displastic kidney (n=3), Juvenile Nephronophitis (n=1), IgA glomerulo-nephritis (n=1). Congenital nephritic syndrome (n=1), Alport syndrome (n=1), Incompatible blood type matching of donor-recipient were A-O (n=4), B-O (n=2), AB – A (n=2), AB-B (n=2),and B-A (n=1). Over all frequency of acute rejection within 3 months after renal transplantation was 50.0% but no humoral rejection were seen in the specimens of episode biopsies. Patient survival and graft survival were 100% and 100% at 5 year. All children are alive and all grafts are functioning until now (16.5m-104.7m). Conclusions: ABO incompatible pediatric renal transplantation should be considered, regarding of excellent outcome.
