Results The admission letters were found to be insufficiently detailed both in terms of disease progression and the patient's present clinical situation. The status of the referring physician was often missing. A discussion of the reasons for this insufficiency led to a proposed model for admission letters which could be used by any physician proposing admission of a patient to a palliative care unit. Using this “standard model” would avoid missing essential information.
