Summary In the hospital setting, aminoglycosides are used for empirical therapy and chemoprophylaxis (although for limited indications in the latter case), either alone or in combination with other antibacterials (β-lactams, glycopeptides and fluoroquinolones). The efficacy of aminoglycosides has been demonstrated in patients with serious infections, mainly those caused by Gram-negative bacteria, methicillin-susceptible staphylococci and some methicillin-resistant staphylococci, Listeria and mycobacteria. Isepamicin (15 mg/kg once daily) is a new aminoglycoside that has been demonstrated to be effective, either as monotherapy or in combination with other antibacterials, in the treatment of respiratory tract, urinary tract, abdominal and cutaneous infections. An 86% to 95% positive response rate was recorded in clinical studies. Eradication rates, both for Gram-negative and Gram-positive bacteria, range from 76% to 100% (mean 90%). Clinical and bacteriological results are similar to those obtained with amikacin, but with a better tolerability profile.
