Study on retropertoneoscopic lymphatic vessel ligation of renal pedicle in the treatment of the galacturia

Objective To investigate the applied value of retroperitoneoscopic lymphatic vessel ligatian of renal pedicle in the treatment of the galacturia.Methods From January 2001 to April 2004,39 cases with galacturia received renal lymphatic vessel ligation were analyzed retrospectively.These cases were diveded into retroperitoneoscopic operation group(group A) and traditional open operation group(group B) .The group A was 21 cases and 24 laterals and group B was 18 cases and 19 laterals.The operative efficacy,operative time,intra and postoperative complications,intraoperative lose blood volume,recovered time of postoperative gastrointestinal fumction and postoperative hospitalization time etc were compared between two groups.Results 39 cases were operative success.Postoperative recovery was smooth and galacturia was disappeared.Group A:1 case happened subcutaneous emphysema in the peri-incision and it was absorbed completely 3 days after operation,4 cases appeared naked eye haematuria after operation and it was disappeared 3 days later.Group B:when the left renal pedicle was separated,the renal arterial extima was damaged to induce formation of aneurysm in one case and the aneurysm was disappeared after renal arterial repair.Right renal vein was damaged during operation and it was repaired successfully.8 cases appeared naked eye haematuria and it was disappeared 2～3 days later.There was wound infection in one case.5 cases complained pain and discomfort in the affected lateral.All cases were followed-up more than 18 months.Recurrence of 1 case and 2 cases were found in group A and B respectively.Unilateral operative time was 72.3±21.6 minutes and 156.7±30.9 minutes in group A and B respectively.Intraoperative bleeding volume was 43.7±19.3 ml and 132.6±61.4 ml in group A and B respectively.The staying time of postoperative renal peripheric drainage tube was 1.5±0.5 day and 3.5±1.5 days,drainage volume was 22.4±1.4 ml and 42.3±25.6 ml and postoperative bed rest time was 2.1±1.1 days and 4.6±1.6 days in group A and B respectively.The time of gastrointestinal functional restoration was 24.6±14.4 hours and 48.0±25.1 hours and postoperative hospitalization time was 5.2±12.5 days and 8.5±2.5 days in group A and B respectively.Conclusion Retroperitoneoscopic lymphatic vessel ligation of renal pedicle has mary advantages,such as less trauma,few bleeding,lower postoperative complications and quick recovery etc.It would replace the open surgery.
